Please read all admission information carefully and sign this form electronically below. Updated official transcripts are needed for all coursework attempted and/or completed since your application date.

I understand I may need to enroll in required LIFE core (also referred to as liberal arts and science or general) courses prior to enrolling in the OTA sequence.
I understand to progress into the OTA courses I must meet all requirements specified below:
- Complete a minimum of two courses with a GPA of 2.75 or better. No grade may be lower than a C. This will help you raise your overall GPA to 2.5, as required by the OTA Online program. This can be completed by either:
  - Completing two or more courses in your first term with a GPA of 2.75 or better
  - Or maintaining a GPA of 2.75 or better for your first two terms of enrollment
- Complete all 4 prerequisite courses with a grade of C or better
- Earn a minimum cumulative GPA of 2.5 or higher
- Earn and maintain an overall GPA of at least 2.5
- Work with your academic success coach to develop and complete a success plan.
- Complete orientations

After you complete the required next steps, you will receive a program plan from your admissions counselor. This plan will outline your projected sequence of courses, as well as your target date to begin the professional sequence of OTA courses. Please understand that any changes you seek to make to the plan going forward may affect your targeted OTA start date. Plans are built on a first come, first serve basis, therefore students who submit their acceptance letter and deposits first will receive their plans first. Please refer to your admissions counselor with additional questions.

I accept this offer of admission. If my academic plans change I will notify the Admission Office in writing. I will also notify the Admission Office of any changes in my name, address or telephone number.

Information regarding registration and orientation will be sent to you after we receive your signed offer of admission and deposit.
